Family members and other trusted supporters are welcome to contact us with questions or to begin exploring services.

Individuals

Looking for Supported Living Services for yourself?You can contact ABCD directly to learn more about our services and referral process.

Getting connected can begin with anyone involved in the person’s support.

Regional Center Service Coordinators

Service coordinators may contact ABCD regarding referrals, service availability, assessments, and coordination of authorized Supported Living Services

Other Support Professionals

Other professionals involved in an individuals support may contact ABCD to begin a conversation or coordinate with the appropriate members of the persons support team

What Happens After You Contact Us?

A referral is the beginning of the conversation—not a commitment to services.

Step 1. We Review the Referral

We learn about the individual, their current living situation, goals, authorized services, and the type of support being requested.

Step 2. We Connect With the Support Team

When appropriate, ABCD coordinates with the individual, Regional Center service coordinator, family, and others the individual chooses to involve.

Step 3. We Meet & Assess

We take time to understand routines, preferences, strengths, support needs, home environment, and personal goals.

Step 4. We Determine the Right Fit

If ABCD and the individual agree that services are a good fit and authorization is in place, we begin developing the individualized support plan and preparing for services.
